Care That Makes a Difference
Compassionate
Every visit starts with respect for the person, not the diagnosis.
Reliable
The visits we promised, the team you already know, and a call back the same day.
Personalized
A plan of care built around your goals and your home — not a template.
At Home
Progress is easier to keep in the place you already know by heart.
Six ways we care for you at home
Most patients need more than one. A nurse manages the medications, a therapist rebuilds the walking, an aide makes the mornings easier — and everyone works from the same plan of care.
- 01
Skilled Nursing
Registered nurses manage medications, wounds and complex conditions in the home — and catch small changes before they become hospital stays.
- 02
Physical Therapy
Rebuild strength, balance and mobility with a program built around the rooms and stairs of your own home.
- 03
Occupational Therapy
Return to dressing, bathing and cooking safely, with practical adjustments that protect independence.
- 04
Speech Therapy
Support for speech, memory and swallowing after a stroke, surgery or neurological diagnosis.
- 05
Home Health Aide
Compassionate help with bathing, grooming and daily routines, supervised by our clinical team.
- 06
Medical Social Services
Guidance through benefits, community resources and the decisions families face — so you are not sorting it out alone.
What to expect
From the first call to the last visit
Starting home health should not feel like paperwork. Here is exactly how it goes.
- 1
Reach Out
Call us, or have the physician send the order. The insurance questions get answered on that first call — not three days later.
- 2
Assessment
A nurse comes to the home, reviews every medication, and looks at the real obstacles: the stairs, the shower, the loose rug.
- 3
Plan of Care
The physician approves a plan built on specific goals — walking to the mailbox, managing the insulin, sleeping through the night.
- 4
Care Begins
Visits start on the agreed schedule, with a team you can name and recognize at the door.
- 5
Ongoing Support
We report progress back to the physician and adjust the plan as things change — in either direction.

The Caregiver Academy
The family does most of the caring. We make sure they know how.
Our clinicians visit a few hours a week. A daughter, a husband, a neighbor covers the rest. So we wrote the answers down: 33 questions families actually ask, answered in full — what to watch for, what is normal, and when to pick up the phone. Free, and nothing to sign up for.

For referring professionals
Send the referral. We take it from there.
You need to know the patient landed safely and that you will hear back. We confirm receipt, handle the benefit check, and keep the referring office in the loop from the start-of-care visit through discharge.

Questions families actually ask
If yours is not here, it is worth a phone call. We would rather answer it straight than have you guess.
(239) 270-5498What is home health care, exactly?
Skilled medical care delivered in your own home under a physician's plan of care: nursing, physical therapy, occupational therapy, speech therapy, home health aide support and medical social services. It is different from non-medical companion or homemaker help, which does not require a physician's order.
Who qualifies for home health care?
In general, a person needs a physician's order, a skilled need such as nursing or therapy, and to be considered homebound — meaning that leaving home takes considerable effort. Every situation is different. Call us and we will review yours before anything is committed.
Is home health care covered by insurance?
Medicare, Medicaid and most private plans cover home health services when the eligibility criteria are met and care is provided by an agency approved under that plan. We verify your benefits and confirm what is covered before care begins, so there are no surprises later.
Do I need a doctor’s order to start?
Yes. Skilled home health requires a physician's order and an approved plan of care. If you do not have one yet, call us — we coordinate directly with physicians' offices to put it in place.
How soon can care begin?
Once we have the physician’s order and your benefits are verified, we schedule the start-of-care assessment as quickly as your situation allows, and hospital discharges are prioritized. Call us with the timeline you are working with and we will tell you honestly what is possible.
¿Atienden en español?
Sí. Contamos con personal que habla español para que usted y su familia entiendan cada paso del plan de cuidado, desde la primera llamada hasta el alta.
Not sure whether home health is the right step?
That is the most common reason people call us, and the call costs nothing. Tell us what is going on and we will tell you plainly whether we can help — or who can.
Monday – Friday, 9:00 AM – 5:00 PM · After hours, the 24/7 clinical line is answered at (239) 645-2730.
